Vendor Information

Thank you for your interest in becoming a Mountain View School District Vendor. All new vendors must fill out a New Vendor Form and W-9 Form. MVSD requires a W-9 Form from all vendors.

Out of State Vendors you may also want to fill out either a CA Form 588 or a CA Form 590. Otherwise we must withhold 7% of your payment to remit to the state. You may receive some or all of that withheld income after you file your California tax return.
