Nutrition Services offers a variety of meal options to support school activities and special events. To help us plan and serve meals efficiently, please complete the appropriate request form below.

  • Field Trip Meal Request Form: Submit this form when planning a class or school outing to ensure that all students have access to a nutritious meal while off campus.
  • Catering Request Form: Use this form to request food and beverage services for school meetings, trainings, celebrations, or other on-site events.
  • Pizza Request Form: Submit this form to order school-approved pizza for classroom rewards, special events, or student activities.

Please follow the submission deadlines noted on each form to allow adequate time for preparation and delivery. If you have any questions or need assistance, contact the Nutrition Services department.

  • USDA NONDISCRIMINATION STATEMENT

Additionally, program information may be made available in languages other than English.

    To file a program discrimination complaint, complete the USDA Program Discrimination Complaint Form, AD-3027 [3 (PDF), found online at How to File a Program Discrimination Complaint and at any USDA office or write a letter addressed to USDA and provide in the letter all of the information requested in the form. To request a copy of the complaint form, call 866-632-9992. Submit your completed form or letter to the USDA by:
